The AFS Portal:
The AFS Portal is a web-based system that allows AFS families to update their contact information, re-enroll, set-up payment plans and pay bills, access & upload medical forms, search for community member contacts and view grades and comments, all within one secure system.
Health Forms:
Starting on July 1, the required medical forms for new and returning students will be made available through Magnus Health, found in the AFS Portal. The deadline to return forms is August 1.
In order to attend the first day of school, all students regardless of grade, must have a copy of their current immunization record on file. If you have questions about required health forms, please contact rmarcus@abingtonfriends.net.

If you haven’t been receiving AFS emails, please be sure your information is up-to-date in the AFS Portal. If it is correct, please contact communications@abingtonfriends.net.

Health Forms for New Students
Tuberculosis Assessment Form
Required for all new students prior to the start of the school year.
DownloadPhysical Examination Form
Required for all new students from Preschool to grade 4.
For grades 5 - 12 the submission of a current Athletics (PIAA) Physical Form meets the requirement.
Medication Permission
Required for any over-the-counter or prescription medications to be administered to your student during school hours.

Required for all new students, preschool to grade 12, prior to the start of the school year.
This form is completed by the parent or guardian and does not require a doctor's signature.
Athletics Physical (PIAA) Form
